The HSG is an early intervention grant programme, funded by the Welsh Government. The programme has been set up to help some of Wales’ most vulnerable people to live independently in their own home or supported housing.

Scheme: The Moorlands Supported Accommodation and Outreach

  • Eligibility - the service user must meet the following eligibility criteria: 
    • Must have a diagnosed mild to moderate learning disability and/or be a vulnerable adult with borderline learning disability
    • Must not require 24-hour support 
    • Must be in need of eligible housing-related support  
    • Must be willing to participate in daytime activities 
    • Must be willing to be supported within a network model
  • Scheme capacity: 10 (accommodation), 45 (outreach)

Service description

The Moorlands supported self-contained accommodation – to assist contract holders with the skills to move on successfully to accommodation and to live independently.

Moorlands Outreach – to help those with vulnerabilities to maintain their accommodation, providing support with benefits, utilities and any other issues to prevent homelessness.

Scheme: Supported Living Accommodation

  • Eligibility:    
    • People with learning or physical disabilities / neurodiversity / mental health / sensory needs.
    • Young people and adults 16 years+, and transitional work with 15-year-olds planning a move into Supported Living accommodation.
    • For people requiring housing-related support confirmed within a personal delivery plan, who have an occupation contract or right to occupy. 

Service description

Supported permanent accommodation. Mixed models of support: non-24-hour / floating support; 24-hour with sleep-in; enhanced 24-hour with sleep-in / live night.
